Job Summary The Nurse Practitioner - Advanced Lung Transplant (Inpatient) is a licensed, certified, non-physician healthcare provider who delivers specialized care to patients undergoing evaluation, transplantation, and post-operative management for lung transplantation. This role includes prescriptive authority and is credentialed and privileged as an independent provider recognized by Medicare and Medicaid. This Advanced Practice Provider works at Baylor University Medical Center in Dallas and focuses on the inpatient management of complex lung transplant patients. The NP collaborates closely with transplant pulmonologists, cardiothoracic surgeons, intensivists, and multidisciplinary teams to provide comprehensive, high-acuity care throughout the transplant continuum. Essential Functions of the Role * Performs comprehensive and focused assessments of patients undergoing lung transplant evaluation and post-transplant care, including detailed medical histories and physical examinations. * Manages complex inpatient lung transplant patients, including ventilator management, immunosuppressive therapy, and post-operative recovery. * Orders, collects, interprets, and evaluates laboratory and diagnostic studies, including pulmonary function tests, imaging, and transplant-specific monitoring. * Prescribes and manages medications, including immunosuppressants, antimicrobials, and supportive therapies, ensuring adherence to transplant protocols. * Develops and implements evidence-based, individualized treatment plans in collaboration with transplant physicians and multidisciplinary teams. * Monitors for complications such as acute rejection, infection, primary graft dysfunction, and organ failure, initiating timely interventions. * Provides daily inpatient management, including hospital rounding, coordination of care, and discharge planning. * Educates patients and families on transplant processes, medication adherence, lifestyle modifications, and long-term follow-up care. * Coordinates care with respiratory therapy, pharmacy, case management, and outpatient transplant teams to ensure seamless transitions of care. * Maintains accurate, timely, and compliant documentation of patient care and clinical outcomes. * Initiates consultations and referrals when patient needs exceed scope of practice or require additional specialty care. * Participates in transplant call coverage and may provide care beyond regular schedules based on program needs. Key Success Factors * Advanced knowledge of lung transplant medicine, including patient selection, perioperative care, and long-term management. * Strong understanding of pulmonary and critical care principles, including ventilator management and respiratory failure. * Knowledge of immunosuppressive therapies and transplant-related complications. * Excellent verbal and written communication skills. * Strong interpersonal and multidisciplinary collaboration skills. * Advanced critical thinking and complex problem-solving abilities in high-acuity settings. * Proficiency in electronic medical records and healthcare technology. * Ability to develop and implement comprehensive, outcomes-based care plans. * Skill in interpreting complex laboratory, imaging, and pulmonary diagnostic data. Qualifications * Texas RN licensure or RN licensure with compact privilege from a Nurse Licensure Compact state, along with Texas APRN authorization. * Current license to practice as a Nurse Practitioner in the state of Texas. * Basic Life Support (BLS) certification within 30 days of hire or transfer. * Certification from an accredited board as an APRN in the applicable patient population required. * *Preferred:* Experience in lung transplant, pulmonary/critical care, cardiothoracic surgery, or inpatient acute care settings.
