Provide skilled nursing assessments to participants. This position is responsible for working directly with the Interdisciplinary Team (IDT), participant, and his or her representative to achieve the goals of the care plan. Communicates to the family or care giver and provides appropriate patient education.
Must have:
Graduate of an accredited Registered Nurse Program and licensed as a professional in the State of Colorado is required.
Health care experience in hospital, long-term or home health care setting with 1 year of experience with a frail or elderly population is preferred.
Must have a reliable transportation and must maintain current automobile insurance.
Must be familiar and work efficiently with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records
Knowledge of medical equipment and instruments
