We’re seeking a Nurse Practitioner (Specialty – Nephrology) to join our mission-driven team of clinicians redefining advanced primary and specialty care. In this full-time role, you’ll support patients across Upland, West Covina, and Apple Valley — and help lead the way in compassionate, whole-person nephrology and dialysis care.
What You’ll Do
Deliver Specialty Clinical Care
- Provide direct, evidence-based care to nephrology patients and those receiving dialysis.
- Lead physical assessments, specialized risk evaluations, and therapeutic decision-making.
- Formulate and document patient care plans in collaboration with supervising physicians.
- Order and interpret diagnostic tests, prescribe medications and treatment therapies.
Champion Risk and Documentation Integrity
- Act as a clinical expert for in-the-moment risk assessments and accurate patient documentation.
- Guide complex patients with a focus on long-term kidney health and dialysis quality.
Be a Regional Clinical Resource
- Rotate across CareMore’s regional practices to meet clinic demand and support integrated care.
What You Bring
Education & Licensure
- Master’s Degree in Nursing (MSN)
- Active, unrestricted RN and NP license in California
- Nurse Practitioner Board Certification
- Minimum 1 year in nephrology or dialysis-focused specialty care
Experience
- 2+ years in a nurse practitioner role, ideally in Internal Medicine, Primary Care, or Geriatrics
- Comfort managing complex, comorbid adult populations
- Bilingual in English and Spanish a plus
Compensation & Schedule
- Compensation: $126,984 – $190,476 + Bonus Eligible
- Schedule: Monday – Friday | 8:00 AM – 5:00 PM
- Travel: Required across clinic sites in Upland, West Covina, Apple Valley, and occasionally to other CareMore locations based on business needs
