- Hospice Registered Nurse Case Manager
Description
The Registered Nurse (RN) Case Manager is responsible for the coordination and delivery of comprehensive hospice care to patients with life-limiting illnesses. This role provides skilled nursing assessment, care planning, symptom management, and patient/family education in accordance with the hospice philosophy of comfort, dignity, and quality of life.
The RN Case Manager serves as the primary clinical point of contact for assigned patients and collaborates closely with the interdisciplinary team to ensure individualized, patient-centered care is delivered across all settings, including private homes and residential facilities.
Key Responsibilities:
Conduct initial and ongoing comprehensive nursing assessments of hospice patients
Develop, implement, and revise individualized plans of care in collaboration with the interdisciplinary team
Provide expert symptom management, including pain and end-of-life care interventions
Coordinate care across disciplines, ensuring continuity and responsiveness to patient needs
Educate patients and families regarding disease progression, medications, and end-of-life expectations
Provide emotional support and guidance to patients and families throughout the hospice journey
Maintain accurate, timely documentation in compliance with Medicare Conditions of Participation and agency standards
Participate in on-call rotation and respond to urgent patient needs as appropriate
Ensure care is delivered in alignment with hospice philosophy, regulatory requirements, and best practices

Requirements
Licensure & Certification:
Current, active, and unrestricted Registered Nurse (RN) license in the State of Arizona
Valid driver’s license with reliable transportation and current auto insurance
Experience:
Minimum of 1 year of nursing experience preferred
Hospice, palliative care, or home health experience is strongly preferred
Newer nurses may be considered if they demonstrate strong emotional intelligence, compassion, and a genuine commitment to end-of-life care
Clinical Competencies:
Strong assessment, critical thinking, and clinical judgment skills
Ability to manage symptoms and provide comfort-focused care
Ability to work independently in home and community-based settings
Knowledge of hospice regulations and Medicare Conditions of Participation is a plus
Communication & Interdisciplinary Collaboration:
Excellent communication skills with patients, families, and interdisciplinary team members
Ability to navigate sensitive and emotional conversations with empathy and professionalism
Experience working within or willingness to embrace an interdisciplinary team approach
Organizational Skills:
Effective time management and ability to prioritize patient needs
Ability to manage a caseload while maintaining high-quality care
Timely and accurate documentation

Education:
Graduate of an accredited nursing program required
Additional Qualifications (Preferred):
Bilingual (English/Spanish)
CHPN certification or interest in obtaining certification
