Lead Care Coordination transformation and advance patient flow across St. Charles Health System in beautiful Bend, Oregon!
The Position
The Nurse Manager, Care Coordination will provide systemwide leadership for Care Coordination across St. Charles Health System, with oversight of RN Case Managers and Case Management Liaisons supporting four hospital campuses.
Reporting to the Director of Care Coordination and Patient Flow, this leader will be primarily based at St. Charles Bend and will travel to the system's northern campuses to maintain a visible leadership presence.
Key priorities include optimizing Care Coordination operations to improve patient flow, length of stay, and discharge planning while strengthening team performance and staffing. This leader will leverage data and benchmarking to identify opportunities for improvement, develop and support the Case Management team, and partner closely with clinical and operational leaders to drive sustainable change and increase organizational capacity.
The ideal candidate will be an engaging, collaborative leader who excels at building trust, developing teams, and navigating change. This individual will be a strategic thinker and strong communicator who is comfortable challenging the status quo, asking thoughtful questions, and creating accountability while maintaining strong relationships. Experience leading in a unionized environment is preferred.
Requirements
BSN required; Master’s degree in Nursing, Management, or a related field required.
Active OR RN license required, or ability to obtain OR RN licensure.
Accredited Case Manager (ACM) certification through ACMA preferred.
A minimum of five years of clinical nursing experience required, coupled with at least one year of leadership experience.
Three or more years of leadership experience within Utilization Review, Case Management, or Care Coordination in an acute care setting is highly preferred.
